There seems to be no way to use Real Player's own configuration to remove itself from the startup application list. So we have to use HijackThis as startup application management tool.
1. Run C:\Program Files\HijackThis\HijackThis.exe
2. Click "Do a system scan only".
3. Locate the following entry in the scan report and click Fix Checked.
O4 - HKLM\..\Run: [RealTray] C:\Program Files\Real \RealPlayer\RealPlay.exe SYSTEMBOOTHIDEPLAYER
4. Restart your Windows system.
